REACH

REACH offers case management services with the goal of addressing the social-emotional, behavioral, developmental, and academic needs of children. Creating a special focus on these childhood needs contributes to Passage Home’s multi-generational self-sufficiency approach.

Case Management: All enrolled children will receive comprehensive case management to set them up for future success. Case Management includes needs assessments, developmental assessments, social-emotional assessments, connection to resources and direct service providers, advocacy, troubleshooting barriers, and much more.

Motel Connect: Select families get perishable and non-perishable food delivered to their current residence on a weekly basis. We hope to ensure children have access to nutritious foods and reduce the effects of food instability.

Teach4REACH: Select children will be provided tutoring services by a qualified volunteer with the goal of helping them develop the skills to reach their academic goals.

Racial Equity Lens
  • At Passage Home, diversity, equity, and inclusion (DEI) are at the core of our operation, including this initiative. 
  • In 2022, Passage Home hired a consultant to evaluate our DEI policies and create actionable steps to further our work in a meaningful and tangible way. 
  • As a result of working with the DEI Consultant, Passage Home has developed a Diversity, Equity, and Inclusion committee at Passage Home.
  • Passage Home’s commitment to diversity, equity, and inclusion includes having a leadership team (including Board Members) that accurately represents the country’s racial and ethnic diversity. The leadership team at Passage Home, including Board Members, consists of 36% Male and 64% female. Sixty-eight percent of board members identify as Black or African American, and 32% identify as White or Caucasian. Our C-Suite staff are 50% female and 75% individuals of color.
Community Voice
  • Passage Home actively works to include and engage the voices of families and community members it serves in decision-making. 
  • One-third of our Board Members are from each of the following groups: Elected Officials, Private Businesses, and Community Representatives (elected by the community).
  • In December 2021, Passage Home completed a Community Needs Assessment (CNA). As a follow-up to the survey, we hosted two community meetings on 12/20/21 to discuss the results and solicit additional feedback from community members. We also utilized quantitative data by reviewing census information, Passage Home records, and other relevant sources to complete a comprehensive assessment. 
  • Distributed two to four times per year, Passage Home’s Participant Satisfaction Survey captures current participants’ self-reported level of family Self-Sufficiency, satisfaction, and suggestions for program improvement.
Impact
  • 1372 Children provided food support (2019-2020)
  • 197 Children maintained stable housing during the pandemic (2019-2020)
  • 303 Months of hotel payments to make sure children did not go homeless during the pandemic (2019-2020)
Next Steps
  • Grow our staff so that we can serve more families experiencing poverty and homelessness
  • Grow more and stronger partnerships with community partners to ensure our clients have access to all the resources that they need
  • Create a data collection system which collects more meaningful information to the wellbeing of clients, tracks clients through time, and allows case managers more time to focus on clients.
